Attached is an electronic version of a paper mailing sent last week from the chancery that has (hopefully) been received by each of the diocesan parishes. It would be helpful to let us know if you didn’t receive the USPS mailing since we are periodically experiencing problems with the reliability of our postal mailings.
Included in that mailing are two important annual forms that each parish is required to submit in a timely manner as outlined:
- Parish annual information update form - including sexual abuse prevention policy compliance
- Assessment form (for establishment of 2025 assessment level)
Also attached (hyperlinked above) to this email is a fillable version of the parish annual information update form that can be used electronically instead of sending a paper version. Please return that fillable form to me at this email address and I will process it to His Eminence and Omelan Mycyk for their files after it is reviewed. Feel free to also send the required annual parish reports electronically instead of using paper if preferred. These would minimally include:
- 2024 annual parish meeting minutes
- Financial reports concerning 2023
- Pastor’s report
